Instructional Information Sheet for License Applications and Renewals
Any previously used application forms are no longer accepted. If an old application/renewal form is submitted, it will be returned to you by either the General Presbyter's office or the General Overseer's office staff.
All applications must be easily readable, legible, and complete.
No sections may be left blank or unanswered, since all the requested information on the application is pertinent and important for our recordkeeping purposes.

Photographs for ID Cards
Every applicant must submit a photograph on a solid background. The size of the photograph should be 2x2 inches / 5x5 cm. Electronic photos are preferred and can be accepted as JPG or PNG files which are compatible with our ID Card printer software.

Processing of License Applications/Renewals
Applicant should complete the appropriate forms and provide a photograph.
Submit forms, photo, and any other required information
to the State/Regional/National Overseer.
